Child Therapy, Family Therapy and Reunification Services
The purpose of counselling, child therapy, family therapy and reunification services is to support the wellbeing, safety, relationships, development, and preferred futures of children and families.
By signing this form, each parent acknowledges and agrees that:
I consent to Adelaide Narrative Therapy Collective providing therapeutic services to my child/children, including child therapy, family therapy, reunification processes, and where appropriate, play-based and play therapy approaches. I understand that play may be used as a developmentally appropriate way for children to communicate experiences, express emotions, build relationships, and participate in therapy.
I understand that Adelaide Narrative Therapy Collective provides a therapeutic service focused upon the wellbeing, safety, relationships, and development of children and families.
I understand that therapeutic conversations may include discussion of experiences, relationships, areas of disagreement, responsibilities, experiences of harm or safety, and the effects these may have upon children and family relationships.
I understand that the therapist is not engaged to make legal findings or determine disputed facts for a Court. The primary purpose of the service is therapeutic. Adelaide Narrative Therapy Collective may, where appropriate, provide reports, professional opinions, expert evidence, or respond to subpoenas within the practitioner's professional expertise (Separate fees are payable).
I understand that both parents may be invited to provide relevant background information to assist the therapeutic process.
I understand that children require an appropriate degree of privacy and confidentiality in order to participate meaningfully in therapy.
I understand that confidentiality is subject to legal and ethical limitations, including mandatory reporting obligations, risk of harm, subpoenas, and lawful disclosure requirements.
I understand that therapy, family therapy, or reunification work will ordinarily not commence until consent has been received from both parents unless otherwise authorised by law or Court Order.
I understand that Adelaide Narrative Therapy Collective generally adopts a transparent approach to communication and may provide both parents with equivalent information.
I understand that counselling services are charged according to Adelaide Narrative Therapy Collective's current fee schedule. I understand that reports, letters, subpoena responses, file preparation, Court attendance, expert witness services, and other legal or administrative services requested outside the ordinary provision of therapy may attract separate fees.
